Responsibilities:

  • Develops a strong referral network and marketing strategies for an assigned area or department

  • Identifies, develops and measures strategic relationships with partners, organizations and facilities.

  • Serves as a liaison between patient and healthcare providers to coordinate patient eligibility and admissions to provide excellent transition of care. Identifies and resolves barriers to the plan of care that best serves patients and families.

  • Identifies resources needed for patients and families including equipment, financial assistance and patient care needs. Maintains knowledge of census and associated resources to assist in determining the best placement.

  • Prepares and delivers tours proposals and presentations using marketing resources including brochures, data, slides, photographs and reports to create a brand and services awareness.

The ideal candidate will have:

  • A current license as a Registered Nurse in Maine or RN license from a compact state

  • 3 or more years of clinical home care or hospice experience required

  • Thorough knowledge and skill in current nursing practice

  • Successful candidates are organized and skilled in problem solving, clinical assessment and care provision, verbal and written communication, and coordination of care.

  • Requires critical thinking skills, decisive judgment and the ability to work with minimal supervision

  • Knowledgeable and skilled in the provision of patient and family centered, relationship-based care.

  • Knowledge of the Home Care and Hospice Regulatory guidelines and practices.

  • Must be able to work in a stressful environment and take appropriate action.

  • Prior experience collaborating to improve relationships with business partners and improving patient experiences

  • Prior experience with education and presentations as well as being comfortable speaking to groups of people.
